Send Us Your Medical Certification & Documents
How to get your leave paperwork to us: upload it on your case, reply to a case email, or fax it. Includes accepted file types, size limits and what happens after it arrives.
Most leaves need a medical certification, a standard form your healthcare provider fills out. When your case needs one, we send you the blank form and the due date (typically 15 calendar days). It is the most important deadline on your case.
There are several ways to get it to us. Uploading it yourself is the fastest, and it is the only one where you can see straight away that it arrived.
Upload from the portal
- Open your case from the dashboard or My Cases.
- Go to the Documents tab.
- Choose your file and upload it. A clear photo of the signed form taken on your phone is perfectly fine.
We accept PDF, DOC, DOCX, JPG, PNG, GIF, HEIC and TIFF files, up to 100MB each. Photos taken on an iPhone and scans saved as TIFF are converted to PDF for you, so they open properly for everyone who needs to read them.
If your form runs to several pages, select them all at once and leave Combine into one PDF ticked. Insurers want a single document per submission, and the pages are merged in the order shown. You can combine up to 20 files at a time.
Other ways to send it
- Reply to an email about your case. When we email you about your case, replying to that email brings your reply and anything attached to it straight back to the case. This is the easiest option when you are away from the portal.
- Fax. Your provider can fax to 1-888-448-6914. Ask them to write your case number on the cover page so we can match it to the right case.
- Attach it to a message. Attach the file to a message to your leave team from the Messages tab on your case.
A brand new email written from scratch reaches us too, but it does not carry your case number, so it may take a little time to appear on your case. Replying to an email about the case is quicker.
After you send it
Anything we have received shows on the Documents tab of your case. If you can see it there, we have it.
Once your certification has been reviewed and marked received, you also get a confirmation email. If a form comes back incomplete, we tell you exactly what is missing and give you time to fix it before anything is decided.
Running late? Message us before the deadline. Extra time is often possible.
