Most leaves need a medical certification — a standard form your healthcare provider fills out. When your case needs one, we send you the blank form and the due date (typically 15 calendar days). It’s the most important deadline on your case.
Upload from the portal
- Open your case from the dashboard or My Cases.
- Go to the Documents tab.
- Choose your file and upload. A clear photo of the signed form from your phone is perfectly fine.
Other ways in
- Fax — your provider can fax directly to 1-888-448-6914. Ask them to write your case number on the cover page; it attaches to your case automatically.
- Message attachment — attach the file to a message to your leave team from the Messages tab.
Running late? Message us before the deadline — extra time is often possible. If a form comes back incomplete, we’ll tell you exactly what’s missing and give you time to fix it before anything is decided.
