How do I submit my paperwork?

Upload it on your case's Documents tab; a clear photo of the signed form is fine. You can also reply to any email we have sent you about your case, which brings the attachment straight back to the case, or have your provider fax it to 1-888-448-6914 with your case number on the cover page.

However you send it, it shows on the Documents tab once it is on your case.
